A GIFT-IN-KIND is any donation of an item that is part of a fundraisinging
event (auction) in honor of a patient.
Such gifts are tax-deductible while gifts directly to patients are
not. The National Transplant Assistance Fund (NTAF) does not ever take title to assets, goods or services, and does
not authorize any community volunteer to do so on its behalf.
Below is the GIFT-IN-KIND Donation Form from the NTAF GREAT LAKES STEM CELL TRANSPLANT FUND structured in honor
of Peter Earley:
The committee contact shall provide one copy of this
form to the contributor, and send one copy to NTAF. Current IRS regulations require donors to obtain a qualified appraisal
for values in excess of $5,000.00
